This scene both horrified and fascinated me as a kid... Nice to see they made children's pop up books, lobby cards and trading cards of the moment! The 70's were something... haha!
For the longest time, I thought it was Lieutenant Xon (from the never produced ST: Phase II / Saavik's arranged betrothed) who died in the transporter accident. It was actually another Vulcan Commander named Sonak, alongside Vice Admiral Lori Ciana.
Check out Vintage Geek Culture, and Scrambled Molecules , and Vibrant Oxymoron for additional details. From Vintage Geek Culture:
Though she wasn’t given a name in the movie, the woman who died in the transporter accident was played by Susan Sullivan, who played Dr. Banner’s girlfriend in the Incredible Hulk television series. She had no dialogue in the finished film, and we didn’t even see her face clearly - amazing since they hired a well known actress. This is because this character, on screen for five seconds, originally was going to have a way bigger role.
I guess it could have been worse... (HOW HAVE I NOT SEEN THIS COMMERCIAL BEFORE?! And using ST: The Next Generation sets?)

Thanks for the additional info, Tonebone!

It's funny... Mad Magazine's parodies of Star Trek, dating all the way back to the original Series, has had scenes of the transporter malfunctioning and reconstituting Kirk all mixed up, with his head attached to his hips, etc.
